How we handle your information
This website is a practice introduction and scheduling intake, not a confidential patient portal. Last updated August 12, 2026.
What this form is for
The public contact form is for scheduling questions, discovery-call requests, and general practice inquiries. Do not submit detailed medical history, diagnoses, lab results, medication lists, or other sensitive health information here.
For clinical details after you are an established patient, use the practice’s approved secure messaging channel. For a medical emergency, call 911 or go to the nearest emergency department. Do not use this form for urgent or emergency care.
Information we collect
When you submit the contact form we receive the name, email, phone number, interest, and message you provide. The message is delivered by email (SMTP) to kathleennp@treanorwellness.com. We also apply standard anti-spam checks (a hidden honeypot field and a minimum submit-time heuristic).
We do not sell contact submissions. Email and mailbox providers that deliver or store the message act as processors for that delivery.
Retention
Inquiry emails are kept only as long as needed to respond and, if you become a patient, to start a chart through the practice’s clinical systems. To request deletion of a website inquiry, email kathleennp@treanorwellness.com.
